How To Register

Patients wishing to register with our practice may either approach our receptionists who will guide them through the process of registration or telephone the surgery at 0141 800 7020 after 10.30 am. You will be given registration forms and new patient questionnaires to be completed for each patient registering or you can download these.  When registering, please bring two independent documents, one such as a passport or driving license to confirm physical identity and date of birth and one such as a utility bill to confirm address. This is required for each individual registering.

Supply of medicines – if you are on repeat medication please hand in your order form from your previous practice.

Practice Boundary

The Practice looks after patients staying in Barrhead, Neilston, Uplawmoor and some Parkhouse G53 areas (Parkhouse G53 patients to confirm with receptionist if area covered) . We have to set a boundary to limit the distance we travel to visit someone. Patients registered with the practice must live within the practice boundary.

If you are an existing patient and you move house and your new postcode does not match the above, then the Practice will ask you to register with a surgery in your area. You can find details of Doctor’s surgeries in your area by contacting the Health Board on 0141 300 1300.
